Access Your Supports is a Geelong based organisation that provides support and services to eligible participants of the National Disability Insurance Scheme (NDIS). With offices in Geelong, Colac and Mildura and over 190 employees, AYS is dedicated to ensuring participants have choice and control to best assist them to achieve their NDIS goals.
AYS Director Ralph Menchise has brought together a team of experienced professionals who are passionate about positive participant outcomes. Not only does Access Your Supports deliver quality support services and empower participants to achieve their life goals with the services available to them under the NDIS, Access Your Supports will also work closely with participants to implement a plan that will work.
ABOUT THE ROLE
The Support Coordinator position will assist NDIS participants to implement support and services utilising their NDIS plan. In addition, the Support Coordinator will build the confidence and skills of the participant with a focus on exercising choice and control.
Key Responsibilities (to name a few):
- Build and maintain rapport and working relationships with participants, carers, families, and service providers
- Consider each individual’s circumstances and level of complexity
- Efficiently budget and break down participant funding
- High attention to detail and ability to multi task
- Provide a varying level of coordination and transition support for participants
- Provide support matching participants with community based and/or mainstream programs to fulfil participant’s individual needs
ABOUT YOU
The successful candidate will have:
- Strong knowledge and understanding of the National Disability Insurance Scheme (NDIS) and disability industry
- Cert IV or above qualification in case management, community services, disability, or social work, AND/OR, extensive experience in case management, community services or disability
- Valid Australian Drivers License
- Comprehensive car insurance
- Cleared Victorian Working with Children Check (WWCC)
- Cleared NDIS Worker Screen Check
In applying for this role, you will be subject to a National NDIS Worker Screen Check. This check involves applying for a National Criminal Police Check. Successful candidates must have an NDIS Worker Screen Check before commencing.
